Medallion Order Due by July 6th, 2024

Season 27 is winding down and it's time to get your Card Protector/Medallion order in

Below are examples of the three types of Medallions and the ways you can order yours!

Please fill out the form below to order your medallion, or speak with one of your directors, you can pay for them via, Venmo, Zelle, or Cash, all orders and payments are due no later than July 6th to ensure we receive them in time for the Main Event!

If you are going to order multiple Medallions please fill out this form for each Medallion, a confirmation text will be sent to you when we receive them (This will be checked several times a day, you may not receive your text right away. If you do not receive a text within 48 hours please text Jim to ensure we received it)

Winners Circle - No Custom - $10.00

Winners Circle - One sided Custom - $20.00

Winners Circle - Two Sided Custom - $30.00

Award Medallion* (Most Points, Most Wins, Bount Hunter, and Best Average) - $10,00 *Cannot be customized, you will be contacted by Jim if you are eligible for an Award Medallion
